Documents Needed for Hiring a Staff Employee

When hiring a new Staff Employee, submit the following Forms and Additional Documents (Contact the Appropriate Finance Office for Information on the Approval Process):

Forms:

  1. Personnel Transaction Form or PTF (Word) (Fillable PDF*)
  2. Georgetown University Employment Application Form (Word) (Fillable PDF*)
  3. Employee Supplemental Data Form (Word) (Fillable PDF*)
  4. Georgetown Instructions to Completing the I-9 Form (Word)
  5. I-9 Form and Instructions (Link to USCIS Website & I-9 Form)
  6. Federal and/or State Tax Form(s)
  7. Direct Deposit Application (PDF*)

Additional Documents

  1. New Employee's Offer Letter (ask HR Generalist for approved format)
  2. New Employee's Resume and Cover Letter
