Student Checklist for a Return from an MLOA

These are the steps to follow if you wish to return from an MLOA:

  • Notify CAPS that you wish to return to Georgetown from your MLOA.  Once we have received your notice, we will begin tracking your materials.
  • Contact your Dean's Office and notify that office of your intent to pursue reenrollment. Begin any academic planning you may need to do with them. Be sure to ask specifically what your college requires from you in order to return (e.g., documentation of activities while away).
  • CAPS requests that students submit all materials by November 1 for consideration for the spring semester, April 1 for the summer sessions, and June 30 for the fall semester. This ensures that CAPS and the Dean’s office have sufficient time to review the student’s request and re-enroll the student. If materials are received shortly after the relevant deadline, the University will attempt to be flexible and review the student’s request to return for the desired semester. However, if there is missing information and/or the University needs additional time to contact the student’s treatment provider, as discussed below, consideration for a return may be made for the following semester rather than the semester for which they were initially seeking to return.
  • If you are presenting treatment results, please submit to the CAPS MLOA Committee one original copy of the Release of Information form for each mental health service provider whom you have seen during your time away and one for your Dean's Office. Be sure to carefully follow the instructions for completing the form.  If you fax these forms to us, please put a hard copy in the mail so that we also have the originals.
  • Give the mental health service provider you have seen during your time away a copy of the Community Provider Report form. Ask your provider(s) to complete the form and mail it directly to the CAPS MLOA Committee (see contact information).
  • If requested at the time of the leave, please arrange for a letter of reference to be mailed directly to the CAPS MLOA Committee for any paid employment, volunteer work, internships, community service, or productive daily activity in which you have engaged during your time away on MLOA.  Letters of reference should be written on letterhead stationery whenever possible, signed, dated, and list the dates of service, hours per week, attendance record, and the nature and quality of the work or experience.  If it would be awkward to have the letter addressed to CAPS, you can have the letter sent to your dean and ask the dean to forward it to us, or you may find an alternate way to get it to us.
  • If requested to do so at the time of the leave, provide a brief statement (no more than two pages) describing (1) your experience away from Georgetown including the activities undertaken while away, (2) your current understanding of the factors that led to the need for the leave, and the insights you have gained from treatment and time away, and (3) how you plan to ensure a successful return to Georgetown University.
  • If you are submitting a transcript for courses taken while on leave, have it mailed directly to the CAPS MLOA Committee.  (Please note that permission to enroll in courses at another university should be cleared in advance with your dean.)  In the event you are currently taking classes for which a transcript will not be available until after the MLOA deadline, you may instead have each of your professors send a brief letter (to the CAPS MLOA Committee) indicating either your expected grade for the course or your status to date.

Work flow.  Once all of your documentation has been received, the process is completed as follows:

  • MLOA Committee reviews all paperwork
  • Once a student receives a satisfactory review, CAPS will contact the student regarding a check-in visit to review the students’ safety and review their plan for sustained health, including recommendations for ongoing treatment, on or off campus.
  • CAPS staff member submits information to the MLOA Committee
  • Student is notified of the decision
  • CAPS communicates its opinion to the student's academic program
