All new students must complete the 2009 Immunization and Tuberculosis Screening Certificate prior to the beginning of classes. This form was recently updated to clarify those countries at lower risk for tuberculosis exposure. If you have already sent in your form you do not need to resubmit the form. Since the fall of 2008, freshman living in residence halls must now show proof of meningitis vaccination. You may waive this requirement by first reading the Meningitis Fact Sheet and then signing and submitting the Meningitis Vaccine Waiver form. |
