All new students must complete the 2013-2014 Immunization and Tuberculosis Screening Certificate prior to the beginning of classes.  This form was recently updated to clarify those countries at lower risk for tuberculosis exposure.  If you have already sent in your form you do not need to resubmit the form.

Since the fall of 2008, freshman living in residence halls must now show proof of meningitis vaccination.  You may waive this requirement by first reading the Meningitis Fact Sheet and then signing and submitting the Meningitis Vaccine Waiver form.

Georgetown University is interested in supporting the success of its students and the safety and welfare of the community.  Sometimes the dean's offices and students themselves have looked to the Student Health Center (SHC) to make recommendations for a medical leave of absence (MLOA) when a student is unable to function in school for physical health reasons.  Follow the link to the University Policy on Medical Leave of Absence for more information.

Georgetown University Hospital policies in accordance with the District of Columbia Mental Health Information Act have been put into place to oversee the handling of Protected Health Information including a patient's medical records.  Included in the policies is a mandate that all request for medical records from the Student Health Center be submitted on a Medical Records Request Form. Once completed please fax the form to (877) 346-1469.
